Appointment Information

First Visit
Each new patient is asked to complete a health history form before their first visit to NorthEast Oncology Associates. This will help our medical staff make a prompt and accurate assessment during the first visit. Please bring your current insurance cards and any current prescription bottles. Please arrive 20 minutes before your scheduled appointment to complete registration and other paperwork, or print and complete the online forms before your arrival.

Routine Visit
The goal of NorthEast Oncology Associates is to provide compassionate quality healthcare. Our staff are available to see patients by appointment during normal office hours. Please arrive 15 minutes before your scheduled appointment with your insurance cards and all prescription medications.

Cancellation Policy
We request that you call forty-eight (48) hours in advance to cancel or reschedule an appointment. We make every effort to reschedule for the first possible opening, at your convenience. We also ask that you call to cancel as early as possible so we may offer these appointments to other patients. Failure to keep an appointment without prior notification may result in a charge for that visit.
